Important Details Regarding Your Personal Health Information

For all Ontarians The Ontario Laboratories Information System (OLIS) is an electronic system that allows hospital and
community laboratories to share your laboratory test results with your health care providers within Ontario. AS Of mid-
December 2010 the Ministry of Health (MOH) assumed custody and control of OLIS, which will be used by laboratories and
health care providers to electronically transmit and share your laboratory test results. If you wish to withdraw your consent
for your personal health information in OLIS to be disclosed to MOH and health care providers, please call 1-800-291-1406
(TTY: 1-800-268-7095) or visit www.ontario.ca/yourhealthprivacy for more information.

For the ON Health Card, the removal of donor approval is as follows: You can change your mind about your organ donation preferences at any time. You can withdraw your consent or change any exceptions you would like to make in your consent profile by logging on to the website ServiceOntario.ca/BeADonor or mailing in a Gift of Life consent form or visiting any Service Ontario centre.
